ABSTRACT

O presente artigo problematiza aspectos afetivos inerentes à relação materno-filial que podem estar associados ao surgimento e estabelecimento de sintomas psicossomáticos de refluxo gastroesofágico no bebê de até 1 ano de idade. Para tanto, apresenta-se estudo de caso de uma díade mãe-bebê auxiliado por entrevista semiestruturada, aplicação das pranchas 1, 2 e 7MF do teste de apercepção temática e observação naturalista. Cada instrumento foi analisado qualitativamente e teve seus resultados integrados e articulados à teoria psicanalítica. Os principais resultados apontaram certa fragilidade egóica e necessidade de apoio social por parte da mãe, compatíveis com o período do puerpério. São discutidas possíveis maneiras de funcionamento do psiquismo materno, por exemplo, quando sobrecarregado com afetos ansiosos, há sobredeterminação de sintomas psicofuncionais no bebê, os quais, por sua vez, causam efeitos no modo como a mãe se posiciona no exercício da maternagem suficientemente boa, marcando um interjogo relacional


This article discusses affective aspects inherent to mother-child relations that may be associated with the onset and establishment of psychosomatic gastroesophageal reflux symptoms in infants up to 1 year old. A case study of a mother-child dyad was performed by conducting semi-structured interviews, applying the 1, 2 and 7MF cards of the Thematic Apperception Test and using naturalistic observation. Instruments were analyzed qualitatively and their results were integrated and linked to psychoanalytic theoretical framework. Results pointed to a certain egoic fragility and the need for social support for the mother compatible with the puerperium. It also discusses possible ways in which the maternal psyche functions. When overloaded with anxious affections, for example, it overdetermines psychofunctional symptoms in the baby which, in turn, affects how the mother positions herself in the exercise of good maternity, marking a relational interplay


Cet article traite des aspects affectifs inhérents aux relations mère-enfant qui peuvent être associés à l'émergence et à l'installation de symptômes psychosomatiques de reflux gastro-œsophagien chez les nourrissons jusqu'à l'âge d'un an. Une étude de cas d'une dyade mère-enfant a été réalisée en menant des entretiens semi-structurés, en appliquant les planches 1, 2 et 7MF du Test d'Aperception Thématique et en utilisant l'observation naturaliste. Les instruments ont été analysés qualitativement et leurs résultats ont été intégrés et reliés au cadre théorique psychanalytique. Les résultats ont mis en évidence une certaine fragilité égoïque et le besoin d'un soutien social pour la mère compatible avec la puerpéralité. L'étude aborde également les modes de fonctionnement possibles de la psyché maternelle. Lorsque celle-ci est surchargée d'affections anxieuses, elle surdétermine des symptômes psychofonctionnels chez le bébé qui, à leur tour, affectent la façon dont la mère se positionne dans l'exercice d'une maternité suffisamment bonne, marquant une interaction relationnelle


Este artículo discute aspectos afectivos inherentes a la relación madre-hijo que pueden estar asociados con la aparición y establecimiento de síntomas psicosomáticos del reflujo gastroesofágico en el bebé de hasta 1 año de edad. Para ello, se realiza un estudio de caso de una díada madre-hijo, con la aplicación de entrevistas semiestructuradas, con el uso de los tableros 1, 2 y 7MF del Test de Apercepción Temática y una observación naturalista. Los instrumentos se analizaron cualitativamente, y sus resultados se integraron y vincularon al marco teórico psicoanalítico. Los principales resultados apuntaban a una cierta fragilidad egoica y la necesidad de apoyo social de la madre, compatibles con el puerperio. Se discuten posibles formas de funcionamiento de la psique materna, por ejemplo, cuando se sobrecarga de afectos ansiosos, sobredeterminando síntomas psicofuncionales en el bebé que, a su vez, provocan efectos sobre cómo se siente la madre en el ejercicio de una maternaje suficiente buena, marcando una interacción relacional

Introduction: Gastroesophageal reflux disease (GERD) is a highly prevalent condition in the Western world. It is a frequent cause of medical appointments and has a high impact on quality of life (QOL). This study aimed to assess health-related quality of life (HQ-QOL) of patients with non-erosive gastroesophageal disease (NERD) and compare their scores with the scores of patients with GERD symptoms and non-pathological acid reflux (non-NERD). Methods: We studied 42 patients with GERD symptoms­21 with positive pHmetry for acid reflux (NERD group) and 21 with negative pHmetry for acid reflux (non-NERD group). We enrolled patients matched by age and gender. Patient inclusion criteria were based on clinical and endoscopic findings and pHmetry. All other major diseases having an impact on QOL were excluded as well as patients in regular use of proton pump inhibitors, prokinetics, histamine-2 receptor antagonists and anti-depressants. All subjects were asked to fill in a validated translation of the SF-36 questionnaire. We described scores trough median and interquartile range (IQR) and assessed the significance of the comparisons through Mann-Whitney test and chi-square test or Student's t-test when appropriate. Results: We found no significant differences in the eight domains of SF-36 between the two groups. Conclusion: QOL of patients with GERD symptoms was equivalent, regardless of acid reflux at pHmetry (AU)

Background Heartburn and regurgitation frequently occur in the third trimester of pregnancy, but their impact on quality of life has not been thoroughly investigated. Objective To measure health-related quality of life of third-trimester pregnant women with heartburn and regurgitation. Methods Data on obstetric history, heartburn and regurgitation frequency and intensity, history of heartburn and regurgitation and health-related quality of life were collected of 82 third-trimester pregnant women. Results Sixty-two (76%) women had heartburn, and 58 (71%), regurgitation; 20 were asymptomatic. Mean gestational age was 33.8±3.7 weeks; 35 (43%) women had a family history of heartburn and/or regurgitation, and 57 (70%) were asymptomatic before pregnancy. The following quality of life concepts were significantly reduced: physical problems and social functioning for heartburn; physical problems and emotional functioning for regurgitation. There was agreement between heartburn in present and previous pregnancies. Conclusion Heartburn and/or regurgitation affected health-related quality of life of third trimester pregnant women .


Contexto A pirose e a regurgitação ocorrem com frequência no terceiro trimestre de gestação, mas o seu impacto na qualidade de vida não foi completamente investigado. Objetivo Avaliar a qualidade da vida de gestantes do terceiro trimestre com pirose e regurgitação. Métodos Os dados sobre a história obstétrica, frequência, intensidade e história de pirose e regurgitação, bem como a qualidade de vida foram coletados de 82 mulheres do terceiro trimestre de gestação. Resultados Sessenta e duas (76%) mulheres tinham pirose e, 58 (71%), regurgitação; 20 eram assintomáticas. A idade gestacional média foi de 33,8 ± 3,7 semanas; 35 (43%) mulheres tinham história familiar de pirose e/ou regurgitação e 57 (70%) eram assintomáticos antes da gestação. Os seguintes domínios de qualidade de vida estavam significativamente reduzidos: limitação física e aspectos sociais pela pirose; limitação física e aspectos emocionais pela regurgitação. Houve concordância entre pirose nas gestações atuais e prévias. Conclusão A pirose e/ou regurgitação afetam a qualidade de vida de gestantes de terceiro trimestre. .

Objetivo: Materiales y Métodos: La observación de los niños con ERGE y sus familias nos permitió iniciar la comprensión inherente a la enfermedad y todos los temores, y expectativas. Por lo tanto, el conflicto emocional debido a la pérdida de control del mundo externo, sus regularidades y predicciones es evidente, y esto conduce al aumento de ansiedad cuando se enfrentan a la enfermedad física del niño. Durante las entrevistas semiestructuradas llevadas a cabo con las familias de los niños con ERGE, se han encontrado características recurrentes que aluden a una serie de manifestaciones propias de la persona cuando es amenazada por factores en los que no tiene el control. En ese sentido, se observan una serie de defensas que son convenientes en ese momento para soportar el dolor y la incapacidad que la enfermedad del niño representa. Entre las defensas más notables están la negación, la disociación, la intelectualización, la proyección y la regresión, manifestaciones cuyo componente principal es la pérdida de control y la incapacidad de proporcionar al niño la protección que él requiere. Por lo tanto, el conocimiento que el pediatra tiene sobre las capacidades psicológicas del niño y la familia, así como el reconocimiento de la amenaza emocional que la enfermedad implica a los padres, se traducirá en un apoyo adicional a fin de proteger la salud y hacer el proceso un poco más tolerable.


Objective: To describe the psychological characteristics in families of children withGERD. Materials and Methods.Thus, the emotional conflict due to the loss of control of the external world, its regularities and predictions is evident, and this leads to the anxiety increase when facing the physical illness of the child. During the semistructured interviews carried out with the families of the children with GERD, recurrent characteristics that allude to a series of manifestations proper of the person when is threaten by factors on which does not have control have been found. In that sense, a series of defenses that are fine at that moment for bearing the pain and the incapacity that the child's illness represents are observed. Among the most remarkable defenses are: negation, dissociation, intellectualization, projection and regression, manifestations which main component is the loss of control and the inability to provide the child the protection s/he requires. Therefore, the knowledge that the pediatrician has about the psychological capabilities of both the child and the family, as well as the recognition of the emotional threat that the illness implies to the parents will result in an extra support in order to protect the health and make the process a little more tolerable.

ABSTRACT

JUSTIFICATIVA E OBJETIVOS: A doença do refluxo gastroesofágico (DRGE) acomete frequentemente pacientes acima de 40 anos, é uma afecção crônica decorrente do fluxo retrógrado do conteúdo gastroduodenal para o esôfago ou órgãos adjacentes, levando a sinais esofágicos e extraesofágicos associados ou não a lesões teciduais. Os sintomas mais comuns são pirose, regurgitação, queimação retroesternal com irradiação para a garganta e ou epigástrio; porém podem ocorrer manifestações atípicas como: asma, bronquite, tosse, pneumonia de repetição, rouquidão, sinusite, laringite, otalgia crônicas, halitose, desgaste do esmalte dentário e sensação de nó na garganta. O objetivo deste estudo foi relatar o caso de paciente com epigastralgia e sensação de nó na garganta persistente, após a correção cirúrgica da hérnia de hiato e o uso de medicamentos, porém com melhora do quadro clínico com o uso amitriptilina e psicoterapia.RELATO DO CASO: Paciente do sexo masculino, 27 anos, solteiro, reside com a mãe e relata sintomas típicos e atípicos da DRGE, com exames diagnósticos normais, sem melhora, com intensidade 10 da dor pela escala visual numérica mesmo com medidas preventivas, tratamento cirúrgico e tratamento clínico com famotidina, tramadol e domperidona. A conduta da equipe de controle da dor foi continuar com as medidas preventivas e o tratamento medicamentoso, introduzir codeína, amitriptilina e psicoterapia, pois foi identificado um quadro de conflito com a mãe.CONCLUSÃO: O paciente apresentou melhora significativa da dor com o uso de amitriptilina, o que concorda com a literatura, pois ocorre melhora em 30% dos pacientes com o uso de antidepressivo tipo amitriptilina, em associação com medicamentos específicos usados no refluxo gastroesofágico como a domperidona e omeprazol.


BACKGROUND AND OBJECTIVES: Gastroesophageal reflux disease is common in patients above 40 years of age. It is a chronic affection caused by retrograde flow of gastroduodenal content to the esophagus or adjacent organs, leading to esophageal and extra-esophageal signs, associated or not to tissue injuries. Most common symptoms are pyrosis, regurgitation, retrosternal burning with irradiation to throat and/or epigastrium; however there may be atypical manifestations, such as asthma, bronchitis, cough, repetition pneumonia, hoarseness, sinusitis, laryngitis, chronic earache, halitosis, teeth enamel wear and sensation of knot in throat. This study aimed at reporting the case of a patient with epigastralgia and persistent sensation of knot in throat after surgical hiatal hernia correction and use of drugs, however with improvement of symptoms with amitriptyline and psychotherapy.CASE REPORT: Male patient, 27 years old, single and living with his mother, reporting typical and atypical GERD symptoms, with normal diagnostic tests, without improvement and with pain intensity 10 according to numerical visual scale even with preventive measures, surgical treatment and clinical treatment with famotidine, tramadol and domperidone. Pain control team approach was to maintain preventive measures and drug treatment, to introduce codeine and amitriptyline and psychotherapy because a conflict with his mother was identified.CONCLUSION: Patient had significant pain improvement with amitriptyline, which is in agreement with the literature because there is 30% improvement of patients with the use of amitriptyline-type antidepressants, associated to specific drugs used for gastroesophageal reflux, such as domperidone and omeprazol.

ABSTRACT

OBJECTIVE: The purpose of this study was to establish baseline data for a procedure that has yet to be defined within a Caribbean population. Using a specifically designed postoperative questionnaire, symptomatology and quality of life were assessed before and after laparoscopic Nissen fundoplication. METHODS: Twenty-three consecutive patients of the same surgeon were identified. The questionnaires measured the symptoms and social and emotional functioning, assigning each a score for comparative purposes. The cumulative score was considered the ultimate index of overall quality of life. Wilcoxon matched pairs test was used to analyse the data. RESULTS: The mean age of the patients was 47.5 years. There were 21 females and 2 males. Heartburn (78.3%) and regurgitation (60.8%) were the main symptoms. Postoperatively, these decreased to 17.3% and 4.3% respectively, with significant declines in other complaints such as dysphagia and nausea. The social and emotional functioning score had an average of 13.2 preoperatively; this increased to 18.3, postoperatively, out of a possible maximum of 20. None of the procedures had to be converted to open laparotomy and mean operating time was 108 minutes. Mean follow-up time was 9.7 months. CONCLUSION: Laparoscopic Nissen fundoplication does provide significant improvement in overall quality of life among patients with gastrointestinal symptoms and can be performed effectively within a Caribbean setting.


OBJETIVO: El propósito de este estudio fue establecer los datos básicos para un procedimiento que tiene que ser definido todavía en el contexto de una población caribeña. Usando una encuesta postoperatoria específicamente diseñada, la sintomatología y la calidad de vida fueron evaluadas antes y después de la fundoplicación laparoscópica de Nissen. MÉTODOS: Se identificaron veintitrés pacientes consecutivos del mismo cirujano. Las encuestas midieron los síntomas así como el funcionamiento social y emocional, asignando a cada uno una puntuación a fin de establecer comparaciones. La puntuación cumulativa fue considerada el índice final de la calidad general de vida. La prueba de pares equiparados de Wilcoxon se usó para analizar los datos. RESULTADOS: La edad promedio de los pacientes (21 hembras y 2 varones) fue 47.5 años. La acidez estomacal (78.3%) y la regurgitación (60.8%) fueron los síntomas principales. Postoperatoriamente, éstos disminuyeron a 17.3% y 4.3% respectivamente, con disminuciones significativas en el caso de otras dolencias, tales como la disfagia y la náusea. La puntuación del funcionamiento social y emocional tuvo un promedio de 13.2 preoperatoriamente. Este aumentó a 18.3 postoperatoriamente, de un máximo posible de 20. Ninguno de los procedimientos tuvo que ser convertido a laparotomía abierta y el tiempo promedio de operación fue de 108 minutos. El tiempo promedio de seguimiento fue de 9.7 meses. CONCLUSIÓN: La fundoplicación laparoscópica de Nissen proporciona un mejoramiento significativo de la calidad general de vida entre los pacientes con síntomas GI y puede realizarse de modo efectivo en un contexto caribeño.

ABSTRACT

INTRODUÇÃO: Estudos têm demonstrado ser a doença do refluxo gastroesofágico capaz de alterar a qualidade de vida e a produtividade no trabalho dos doentes por ela acometidos. Instrumentos para esse tipo de avaliação são provenientes, em sua maioria, de países de língua inglesa e/ou francesa. A utilização desses instrumentos em nosso meio demanda criterioso processo de tradução e validação. OBJETIVOS: Traduzir para língua portuguesa os questionários GERD-HRQL (Gastroesophageal Reflux Disease - Health Related Quality of Life), HBQOL (Heartburn Specific Quality of Life Instrument) e GSAS (Gastroesophageal Reflux Disease Symptom Assessment Scale) específicos para avaliação de qualidade de vida na doença do refluxo gastroesofágico. Testar suas propriedades psicométricas de confiabilidade e validade. MÉTODOS: Cento e trinta e dois pacientes com doença do refluxo gastroesofágico (idade média 54,9 anos, ± DP 13,9) atendidos no ambulatório de motilidade digestiva da Universidade Federal de São Paulo, SP, e de gastrocirurgia da Faculdade de Medicina de São José do Rio Preto, SP, aceitaram participar do presente estudo, fornecendo termo de consentimento pós-esclarecimento. Destes, 40 pacientes participaram da fase de pré-teste (28 do sexo feminino e 12 do sexo masculino, com idade média de 55,3 anos, ± DP 14,7) e 92 da fase de validação (64 do sexo feminino e 28 sexo masculino, com idade média 54,7 anos e ± DP 13,7). A tradução e adaptação cultural foi realizada de acordo com o método de GUILLEMIN et al., sendo a validação dos questionários traduzidos (GERD-HRQL, HBQOL e GSAS) realizada em relação aos instrumentos genérico SF-36 e sintomßtico ESDRGE (SQGERD). RESULTADOS: A adaptação cultural implicou na troca de quatro palavras no GERD-HRQL, seis no HBQOL e nove no GSAS. Posteriormente a esta fase, o questionário GSAS foi abandonado por problemas no cálculo do escore, sendo as propriedades de medidas testadas nos dois questionários...


BACKGROUND: Gastroesophageal reflux disease has been shown patients to alter quality of life and working productivity. Most of the instruments available for this type of assessment come from English or French speaking countries. To use these instruments in Brazil requires a judicious process of translation and validation. AIM: Translating to Portuguese the questionnaires GSAS (Gastroesophageal Reflux Disease Symptom Assessment Scale), GERD-HRQL (Gastroesophageal Reflux Disease - Health Related Quality of Life) and HBQOL (Heartburn Specific Quality of Life Instrument) specific for quality of life assessment in gastroesophageal reflux disease. Testing the psychometric properties of reliability and validity of the referred disease specific instruments. METHODS: One hundred and thirty two gastroesophageal reflux disease patients (mean age 54.9 years and ± SD 13.9) from the Digestive Disease Motility Outpatient Clinic, Federal University of São Paulo, SP, Brazil and the Department of Surgical Gastroenterology "São José do Rio Preto" School of Medicine, São José do Rio Preto, SP, Brazil, accepted to participate and signed the informed consent form. Forty of these patients took part in the pre-test phase (28 females and 12 males, mean age 55.3 years ± SD 14.7) and the remaining 92 part in the validation phase (64 females and 28 males, mean age 54.7 years and ± SD 13.7). The translation and cultural adaptation processes were carried out accordingly us to the method of Guillemin et al (1993). The validation processes of the disease specific translated questionnaires (GSAS, GERD-HRQL and HBQOL) was performed in relation to a generic (SF-36) and a symptomatic (SQGERD) instrument. RESULTS: Nine words of the GSAS, four of the GERD-HRQL and six of the HBQOL were replaced during the cultural adaptation phase.
